For the Personalized package, enter the player’s last name and jersey number with your order. Team name, school colors, mascot, and wording will be adapted for your school.

Custom Jersey Photo Required

If you order the Personalized package, you must email a clear photo of the player’s jersey to Twelvefiftydesigns@gmail.com.

Please include the player’s last name and your order number in the email so the photo can be matched to the correct order.
